Salesforce pre-admission assessment form: what in-depth capture looks like, and how to decide what you need

One pre-admission assessment we built in Salesforce now runs to over 250 fields. Plenty of teams want something far smaller. Here is how the big one works, and how to pick the right size for yours.

In short

  • A pre-admission assessment (PAA) form in Salesforce keeps the assessment on the opportunity, where admissions already works.
  • Our in-depth example has over 250 fields, with skip logic so each person only sees the questions that apply.
  • Capture a field only if someone uses it: a decision, a report, an alert or the EMR.
  • Smaller forms are a good choice too. Start from what admissions needs before admit, and leave the rest to the clinical team.

What a pre-admission assessment form in Salesforce does

A pre-admission assessment is the set of questions admissions asks before a patient is admitted. Contact and insurance details, medical history, mental health and substance use history, and what the team needs to decide on the right level of care.

When the assessment lives on paper or in a separate form tool, someone re-types it before admit. Building it in Salesforce puts it on the opportunity, next to the calls, the insurance check and the notes, so the record tells the whole story.

An in-depth example: a PAA form with over 250 fields

One of our builds is a custom pre-admission assessment form in Salesforce that now captures over 250 fields. It is a Lightning Web Component that opens from a quick action on the opportunity, so the admissions rep fills it in without leaving the record.

  • Fields mapped first. The questions from the previous assessment tool were pulled into a Salesforce field mapping sheet before anything was built.
  • Skip logic. Medical sections open only when an earlier answer calls for them, so a long form does not feel long.
  • Required fields highlighted. Missing answers are flagged on the field itself, and the client name is split into first and last name.
  • Mapped to the opportunity. Details already on the opportunity fill the assessment, so nothing is asked twice.

Where the assessment goes after it is filled in

Into the EMR

The completed assessment is sent to Kipu as a PDF on the assessment tab, so the clinical team sees it without re-typing.

Alerts with the PDF

An email to each facility's Teams channel, with the assessment PDF attached, so the right team knows it is ready.

Approval on the record

An "Intake Assessment Approved" checkbox drives the next steps by automation.

Back to the ad platforms

A completed assessment can be sent to Google Ads as an offline conversion. Only the outcome goes, not the answers, diagnosis or treatment.

How to decide what your Salesforce assessment form should capture

Every field has a cost. Someone has to ask it, someone has to answer it, and someone has to keep it accurate. Before a question goes on the form, check it against these.

  • Does it drive a decision before admit, like level of care or insurance fit?
  • Does a report, dashboard or alert use it?
  • Does the EMR need it, so it should travel in the PDF or the sync?
  • Is it needed now, or can the clinical team ask it after admit?
  • Is it already somewhere else on the record, so the form can fill it in?

If a question fails all five, leave it out. If it only matters for some patients, put it behind skip logic instead of showing it to everyone.

When a smaller form is the better choice

Not every intake needs depth. For one referral intake screen, we went the other way and cut the new lead page down to only the fields a referral needs, so reps could enter a referral and move on.

  • A short first form, with a fuller assessment later in the process.
  • Required fields kept to what admissions cannot work without.
  • The same form grown section by section, as reports show what is missing.

Salesforce can be changed in any way, with flows, custom objects, Lightning components and Apex, so a form can start small and grow without a rebuild.

Patient data in a Salesforce assessment form

An assessment holds patient health information. Decide who can see each section, keep the answers out of anything sent to ad platforms, and send the clinical detail to the EMR where it belongs. For healthcare work, we sign BAAs.
